29 CFR 1910.1026(l)(1):     The employer did not ensure that all employees who were assigned to workplaces where there was exposure to chromium (VI) were provided with information and training as required by the Hazard Communication standard, 29 CFR 1910.1200:    a) On or about 12 July 2012, the employer did not provide training to chrome plating operator(s) on the health hazards of Hexavalent Chromium [Cr(VI)] and of  Heff 25 RS1 Chromic Acid (contains Chromium Trioxide) and the signs and symptoms of adverse health effects that may result from Cr(VI) exposure.

29 CFR 1910.1200(h)(1):     The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area.    a) On or about 12 July 2012, the employer did not provide training to chrome plating operator(s) with effective information and training for the hazardous chemicals used in the workplace including but not limited to Hasa Caustic Soda 50% (flakes) (contains Sodium Hydroxide).
