On or about July 18, 2012, employees, servicing equipment throughout the manufacturing area, were exposed to hazards resulting from the unexpected release of stored energy because the employer did not have clear and specific written energy control procedures that outlined the scope, rules and steps for the control of hazardous energy for various machines, such as but not limited to the Ficep A 164T machine.

On or about July 18, 2012, in the FICEP area, employees were using the Dayton 10-inch Bench Grinder to grind steel pieces with the work rest adjusted approximately 3/8-inches away from the grinding wheel, exposing themselves to struck-by hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a.  On or about July 18, 2012, in the FICEP area, employees using the Dayton 10-inch Bench Grinder to grind metal pieces had the adjustable tongue for the left grinding wheel 1 inch away from the grinding wheel, exposing themselves to struck-by hazards.     b.  On or about July 18, 2012, in the FICEP area, employees using the Dayton 10-inch Bench Grinder to grind metal pieces had the adjustable tongue for the right grinding wheel  1-3/16 inches away from the grinding wheel, exposing themselves to struck-by hazards.

Both grinding stones were grooved by the parts being grinded, and exposed the employees to struck-by hazards.
