29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    Job site located at 8710 S 71st Plaza, Papillion, NE 68133-Employees were allowed to work on a scaffold where the scafflod legs were not bearing on the base plate and mud sills or other adequate firm foundation. The legs were set on a mud sill without the base plate (feet) being in place on the legs.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of acces    Job site located at 8710 S 71st Plaza, Papillion, NE 68133-Employees were allowed to climb the legs of the scaffold to access the working areas of the scaffold. The employer did not have a safe means of access and egress from the scaffold work platform such as a ladder or other safe means of access and egress.
