29 CFR 1926.21(b)(5): Employees required to handle or use flammable liquids, gases, or toxic materials were not instructed in the safe handling and use of those materials and were not made aware of the specific requirements contained in Subparts D, F, and other applicable subparts of this part:    a) Destiny USA, on or about 8/18/12: Welders performing hot work were not trained to inspect fire extinguishers as set forth in Subpart F of the construction standard, 29 CFR 1926. Fire equipment was not inspected for use in emergencies involving liquified propane gas (LPG) in that:    1) A fire extinguisher, name and model unknown without markings was last inspected in February of 2008.     2) A Sentry ABC fire extinguisher was not inspected since July of 2005. No serial numbers or markings were discernible. The extinguisher had been discharged.     3) An Amerex model B456, 4A:80-B:C dry chemical fire extinguisher, serial E-554774 was not inspected since May/June of 2011.     4) An Amerex model B456, 4A:80-B:C dry chemical fire extinguisher, serial E-454500 was not inspected since May/June of 2011.     5) An Amerex model B456, 4A:80-B:C dry chemical fire extinguisher, serial E-453126 was not inspected since May/June of 2011.     6) An Amerex model B456, 4A:80-B:C dry chemical fire extinguisher, serial E-453135 was not inspected since May/June of 2011.

No. 10A-1970:    a) Destiny USA, on or about 7/18/12: A fire extinguisher, name and model unknown without markings was last inspected in February of 2008. The extinguisher was provided for hazards resulting from welding operations.    b) Destiny USA, on or about 7/18/12: A Sentry ABC fire extinguisher was not inspected since July of 2005. No serial numbers or markings were discernible. The extinguisher was provided for hazards resulting from welding operations.    c) Destiny USA, on or about 7/18/12: An Amerex model B456, 4A:80-B:C dry chemical fire extinguisher, serial E-554774 was not inspected since May/June of 2011. The extinguisher was provided for hazards resulting from welding operations.    d) Destiny USA, on or about 7/18/12: An Amerex model B456, 4A:80-B:C dry chemical fire extinguisher, serial E-454500 was not inspected since May/June of 2011. The extinguisher was provided for hazards resulting from welding operations.    e) Destiny USA, on or about 7/18/12: An Amerex model B456, 4A:80-B:C dry chemical fire extinguisher, serial E-453126 was not inspected since May/June of 2011. The extinguisher was provided for hazards resulting from welding operations.    f) Destiny USA, on or about 7/18/12: An Amerex model B456, 4A:80-B:C dry chemical fire extinguisher, serial E-453135 was not inspected since May/June of 2011.
