29 CFR 1926.1101(g)(5)(i)[B][2]:  Electrical circuits in the enclosurewere not deactivated, unless equipped with ground-fault circuit interrupters:    a)  On or about June 20, 2012, and at times prior, employees were exposed to electrical shock hazards while they worked inside an asbestos abatement enclosure in which all electrical circuits had not been deactivated or equipped with a ground-fault interrupter.        Abatement Note:  Abatement certification is required for this item (see enclosed Sample Abatement-Certification Letter).

29 CFR 1926.1101(f)(5):  The employer did not, as soon as possible but no later than 5 days after the receipt of the results of any monitoring performed under section, notify each affected employee of the results either individually in writing or by posting the results in an appropriate location that was accessible to employees:    a)  On or about June 20, 2012, and at times prior, the employer did not ensure that employees were provided with the results of exposure monitoring for asbestos.
