29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel:    (a) At 635 Old Hwy. 90 West, Vidor, TX, Crown Section of the fabrication shop, employees were exposed to electric shock when using a E54864 12 AWG extension cord to energize a 120 volt grinder and fan plugged into a Miller welding machine without a ground fault circuit interrupter (GFCI) for employee protection.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage:    (a) At 635 Old Hwy. 90 West, Vidor, TX, Crown Section of the fabrication shop, employees were exposed to electric shock when using a E54864 12 AWG extension cord to energize a 120 volt grinder and fan.  The flexible cord was exposed to hot electrical equipment (welding ground), hot slag, hot metal surfaces (from welding and cutting), pinching from heavy objects, etc.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    (a) At 635 Old Hwy. 90 West, Vidor, TX, Crown Section of the fabrication shop, employees were exposed to electric shock when using a E54864 12 AWG extension cord to energize a 120 volt grinder and fan with no strain relief.    (b) At 635 Old Hwy. 90 West, Vidor, TX, Crown Section of the fabrication shop, employees were exposed to electrical shock when using a ground cable connected to a ground clamp without a strain relief to conduct MIG welding.
