This hole was approx. in the center of the building and the length of the hole ran in the north-south direction. Fabricated pieces of ductwork were stored near the south-east end of the hole which was approx. 15 in. wide x 20 ft. long x 12 in. deep.    b) On or about 7/31/12 inside building under renovation at 175 Jefferson Ave., Buffalo, NY: Employees were exposed to the potential of stepping into a shallow, narrow, rectangular floor hole which was left uncovered. This hole was in a room at the south end of the building, east of the floor hole in instance (a) above. Fabricated pieces of ductwork were stored on the north side of this uncovered hole which ran in the east-west direction and was approx. 15 in. wide x 20 ft. long x 12 in. deep.    NO ABATEMENT CERTIFICATION REQUIRED
