29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a) Doroteo Construction LLC at 2645 E 141st Place, Thornton, CO 80602:  On or before August 4, 2012, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.404(f)(6):  The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) Doroteo Construction LLC at 2645 E 141st Place, Thornton, CO 80602:  On or before August 4, 2012, the employer did not ensure that the orange, red and black, and green extension cords were properly grounded.  These cords were missing their ground plugs, and were used to power 110 volt skilsaws.  Condition exposed employees to electrical shock hazards.

29 CFR 1926.416(e)(1):  Worn or frayed electric cords or cables were used:    a) Doroteo Construction LLC at 2645 E 141st Place, Thornton, CO 80602:  On or before August 4, 2012, the employer provided a 110 volt Skilsaw, serial # HDxxx899, for use, and the blue cord connected to the saw had damaged exterior insulation.  Condition exposed employees to electrical shock hazards.
