Molding Press #62 Platform - On or about August 9, 2012, employees working on an open sided platform approximately 6 feet high, were not provided with an adequate standard railing. Employees were thereby exposed to hazards associated with falls.

Entire facility - On or about August 9, 2012, the employer had not conducted periodic inspections of established energy control procedures in place for employees who perform servicing and maintenance on such equipment including, but not limited to, the injection molding machines. Maintenance Shop - On or about August 9, 2012, the Kalamazoo horizontal band saw had an unused portion of the blade that was not guarded. Employees were thereby exposed to hazards associated with reciprocating saw blades.
