29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  a. Molding Press #62 Platform - On or about August 9, 2012, employees working on an open sided platform approximately 6 feet high, were not provided with an adequate standard railing.  Employees were thereby exposed to hazards associated with falls.

29 CFR 1910.147(c)(6)(i):     The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a. Entire facility - On or about August 9, 2012, the employer had not conducted periodic inspections of established energy control procedures in place for employees who perform servicing and maintenance on such equipment including, but not limited to, the injection molding machines. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation:    a. Maintenance Shop - On or about August 9, 2012, the Kalamazoo horizontal band saw had an unused portion of the blade that was not guarded. Employees were thereby exposed to hazards associated with reciprocating saw blades.
