29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:      (a) Front side of townhouse: An employee was exposed to fall hazards of approximately 26' to the ground while engaged in masonry work on a tubular-welded scaffold with single plank which measured approximately 8" wide x 10' long. as it was resting on a 2x4 face-nailed to the structure.

(a) Front side of townhouse: An employee was exposed to falls of approximately 24' while engaged in masonry work on a tubular welded scaffold system which was erected 4 sections high and not tied into the structure.
