On or about 08/28/12, main access road, entrance Trailer Control Area (TCA), Crowley Puerto Rico Services, Inc.  Isla Grande, San Juan, PR.  At a blind spot/curve before the TCA entrance where the visibility from both ways was impaired, the employer did not install a stop sign and/or warning mirror system. On or about 08/28/12, main access road, entrance Trailer Control Area (TCA), Crowley Puerto Rico Services, Inc.  Isla Grande, San Juan, PR.  The employer did not install traffic controls to inform drivers to operate at safe speeds as evidenced by the lack of posted speed signs, stop signs, and, speed bumps along the access road and blind sections on the road.
