29 CFR 1910.178(l)(1)(i):     The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    Throughout facility - On or about September 4, 2012, the operators were not trained in the safe operation of powered industrial trucks, thereby exposing employees to struck by hazards.

29 CFR 1904.29(a):     Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.    Recordkeeping - On or about September 4, 2012, the employer failed to keep OSHA logs, summaries and injury and illness reports for 2011 and 2012.
