29 CFR 1910.138(a): Employers did not select and require employees to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.    a. Drainage Products, Inc. located in Haviland, Ohio:  On or about September 6, 2012, the employer did not ensure employees performing the coiling operation were wearing hand protection when cutting polyethylene pipe to length.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury, or illness case that resulted in the general recording criteria on the OSHA 300 or equivalent:    a. Drainage Products, Inc. located in Haviland, Ohio:  On or about September 6, 2012, the employer did not record the following work place injuries or illnesses on the OSHA 300 or equivalent.    -An employee  received a second degree burn to his left forearm when working on the screen changer and subsequently received a prescription steroid cream for the injury.    -An employee suffered a crushing injury to the finger on his left hand when it was struck by a travel screw resulting in three lost work days.
