29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:    On or before September 5, 2012 at 11512 Robert Lennox Dr., one employee was working on the top section of the scaffold without any form of fall protection. This condition exposed the employees to a fall hazard of approximately to 12 feet 7 inches to the ground below.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    On or about September 5, 2012, at 11512 Robert Lennox Dr., El Paso, TX, on the north side of the building where one employee was operating from the scaffold not fully planked. This condition exposed the employees to a fall hazard of approximately 12 feet 7 inches to the ground below.    BEST PLASTERING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29CFR 1926.451(B) (1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 307250571, CITATION NUMBER 02, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON  10/15/2009, WITH RESPECT TO A WORKPLACE LOCATED AT 14225 PATRIOT POINT, EL PASO, TX 79927  		  BEST PLASTERING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29CFR 1926.451(B) (1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 307251678, CITATION NUMBER 02, ITEM NUMBER 002, AND WAS AFFIRMED AS A FINAL ORDER ON  09/14/2009, WITH RESPECT TO A WORKPLACE LOCATED AT 460 THUNDER BLUFF, EL PASO, TX 79927

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On or about September 5, 2012, at 11512 Robert Lennox Dr., El Paso, TX, on the north side of the building where one employee was observed climbing the scaffold side to access the scaffold.  This condition exposed the employees to a fall hazard of approximately 12 feet 7 inches to the ground below.
