29 CFR 1915.83(c)(2):  When the vessel was supplied with electric shore power, the employer did not ensure that each circuit to be energized was in safe condition, prior to energizing any of the vessel's circuits:    a)  At the west end of the yard: On September 12, 2012 and at times prior to; the employer failed to ensure that each circuit to be energized was in a safe condition in that the main conductor supplying electrical power to 3, 230, volt switch boxes was rated at 100 amps while each of the switch boxes was capable of drawing 100 amps each, exposing employees to fire hazards such as, but not limited to burns and smoke and fume inhalation.

29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel:    (a)  On the large barge located at the east end of the shipyard: On September 12, 2012 and at times prior to; the employer failed to provide ground fault circuit interrupter (GFCI) protection exposing employees to 110 VAC electrical shock while working with electrical equipment.    Raymond and Associates, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.304(b)(3)(ii)(A), which was contained in OSHA inspection number 315920322, citation number 1, item number 6 and was affirmed as a final order on April 10, 2012, with respect to a workplace located at 14562 Shell Belt Rd., Bayou La Batre, AL  36509.
