29 CFR 1926.451(c)(2)(v):     Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork:      a)  603A Haddonfield Rd., Cherry Hill:  An employee was exposed to fall hazards while working out of a wooden job built waste bin that was not sufficiently attached to the forks of a Lull 844C-42 rough terrain fork truck.   Observed on or about 10/01/12.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
