29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    At the jobsite located at 1278 Day Street, Galesburg, IL, an employee was exposed to potential eye injury from fragmented roofing debris while using a pneumatic nailer and no eye protection was utilized during pneumatic nailing operations.

Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  At the jobsite located at 1278 Day Street, Galesburg, IL, employees were exposed to fall hazards of approximately 10 feet while performing roofing operations over the front entrance to the house without fall protection in place.    b)  At the jobsite located at 1278 Day Street, Galesburg, IL, employees were exposed to fall hazards of approximately 10 feet while performing roofing operations on the back side of the house without fall protection in place.    c)  At the jobsite located at 1278 Day Street, Galesburg, IL, employees were exposed to fall hazards of approximately 9 feet while performing roofing operations on the garage without fall protection in place.    Clay J. Locke dba Locke's Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314847856, citation number 1 , item number 2, group a, and was affirmed as a final order on January 25th, 2011, with respect to a workplace located at 264 Hackberry, Galesburg, IL 61401.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  At the jobsite located at 1278 Day Street, Galesburg, IL, employees were exposed to fall hazards of approximately 10 feet while using the ladder at the front side of the house because the ladder did not extend 3 feet above the landing surface.    b)  At the jobsite located at 1278 Day Street, Galesburg, IL, employees were exposed to fall hazards of approximately 9 feet while using the ladder at the garage because the ladder did not extend 3 feet above the landing surface. Locke dba Locke's Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 314847856, citation number 1 , item number 3, group a, and was affirmed as a final order on January 25th, 2011, with respect to a workplace located at 264 Hackberry, Galesburg, IL 61401.
