29 CFR 1910.23(a)(4): Skylight floor openings and holes were not guarded by a standard skylight screen or a fixed standard railing on all exposed sides:    On or about October 17, 2012, employees were working on the roof which contained approximately 12 skylights which were not guarded by a screen, fixed standard railing, or equivalent means of fall protection.  These employees were exposed to a fall of approximately 30 feet to the concrete floor below.

29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident:     On or about October 17, 2012, the employer failed to report to OSHA a work-related death of an employee which occured approximately on or about October 17, 2012 at approximately 3:00 PM. The OSHA San Antonio Area Office learned of the fatality via media referral on October 18, 2012, at approximately 10:15 AM.
