29 CFR 1926.453(b)(2)(iv):     Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:      (a) As observed on or about 10/18/2012, one employee was observed climbing over the rails of an aerial lift to reach the roof at the construction of a Walgreens at US Highway 41 and Del Prado Blvd. in North Fort Myers. The employee was exposed to a fall of approximately at 26 feet above the ground.

29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:      (a) On or about 10/18/2012, two employees observed working from an aerial lift were not using any kind of fall protection at the construction of a Walgreens at US Highway 41 and Del Prado Blvd. in North Fort Myers. The employees were exposed to a fall of approximately at 26 feet above the ground.
