Wallace Street, Chicago, IL - Employees performing masonry work on the third floor were exposed to falls of approximately nineteen (19) feet, seven (7) inches created by the presence of unprotected side and edges.

Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) On or about October 17, 2012 at the jobsite located at 3526 S. Wallace Street, Chicago, IL - Employees working on the third floor were exposed to falls of approximately nineteen (19) feet due to unprotected window and wall openings. b) On or about October 17, 2012 at the jobsite located at 3526 S. Wallace Street, Chicago, IL - Employees working on the second floor were exposed to falls of approximately six (6) feet seven (7) inches due to unprotected wall openings.
