At the construction site located at 1293 Linwood Ave, Columbus, Ohio, employees were installing brick and accessing the roof from a make shift wooden side bracket of a tubular welded frame scaffold that had wooden scaffold planks that extended perpendicular from the scaffold to the roof, which increased the tipping forces of the scaffold, thereby exposing the employees to a fall hazard of approximately 24 feet.
