Best One Tire & Service of Findlay, Inc. located at 10456 U.S. 224 West, Findlay, Ohio: On or about October 13, 2012, the employer did not ensure the airline being used to service the tire was equipped with an inline pressure guage and/or a regulator. No guages and/or regulators were present inline or on the air compressor.

29 CFR 1910.177(g)(6): Tires on single piece rim wheels were not contained within a restraining device, positioned behind a barrier or bolted on the vehicle with the lug nuts fully tightened during tire inflation:    A. Best One Tire & Service of Findlay, Inc. located at 10456 U.S. 224 West, Findlay, Ohio: On or about October 13, 2012, the employer did not ensure the commercial steering wheel and tire was contained in a restraining device while an employee was servicing a tire with a single piece wheel. While using an air chuck to inflate the tire in an attempt to seat the bead against the wheel flange, the wheel was explosively ejected resulting in serious injuries to an employee.

29 CFR 1910.177(g)(8): The employer did not ensure that employees stay out of the trajectory when inflating a tire on a single piece rim wheel:     A. Best One Tire & Service of Findlay, Inc. located at 10456 U.S. 224 West, Findlay, Ohio: On or about October 13, 2012, the employer did not ensure employees were working outside the trajectory of the tire and wheel. The wheel was explosively ejected out of one side of the tire propelling the tire and wheel upward striking an employee. An employee sustained multiple serious injuries from being struck by the wheel/tire.

29 CFR 1910.177(g)(10): The employer did not ensure that tires on single piece rim wheels were not inflated above the maximum pressure recommended by the manufacturer to seat the tire bead firmly against the rim flange:    A. Best One Tire & Service of Findlay, Inc. located at 10456 U.S. 224 West, Findlay, Ohio: On or about October 13, 2012, the employer did not ensure that the maximum tire pressure of 105 psi was not exceeded while using an air chuck in an attempt to inflate the tire to seat the bead firmly against the rim flange. An in-line pressure of approximately 120 psi was being used to seat the bead when the aluminum wheel was explosively ejected upward.
