29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel. Flight Operations hangar (Bldg 801): On or about 25 October 2012 and times prior thereto, the green Hyster, 393001780954, S/N 657192 had an inoperative horn. This exposed employees to a struck by hazard.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident. Bldg. 628, Main shop area: On or about 25 October 2012 and times prior thereto, 120 volt Circuit Breaker Panel (PNL 628-2) did not have the identification for slot 5, a 20 amp circuit breaker. This exposed employees to electrical shock hazards.
