29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system.    a) Jobsite: 1830 Lombard St., Philadelphia, PA. - On or about 10/24/12, no fall protection system was used to prevent an employee from falling off the edge of the roof of a 12 story building.  The employee was installing a new roof and was within 8 feet from the unprotected flat roof sides and edges.
