29 CFR 1910.177(f)(5):     Tires on multi-piece rim wheels outside of a restraining device were inflated in excess of the pressure required to force the tire bead onto the rim ledge and create an airtight seal with the tire and bead:    a/  Adjacent to the north, exterior wall of the Maintenance Shop, on or about 10/24/12, one employee was directed to use a tire inflation tool to inflate a PRIMEX 42X25-20 16-Ply Tire.  This tire was neither attached to a vehicle nor placed inside of a suitable cage.  Remote control inflation equipment was not provided or used.  The tire measured approximately 48 inches in diameter.  The employee remained in the trajectory of the rim.  This rim suffered a catastrophic failure behind the bead propelling the employee onto the concrete pad.
