29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a.) Horizon of Toledo, LLC dba Liberty Wholesale Auto Parts, LLC located at 1463 N. Michigan St. Toledo, OH; On or about November 14, 2012, the employer did not assure that the man basket used to lift employees to the roof was secured to the lifting carriage of the Mitsubishi fork lift. On November 6, 2012, an employee fell approximately 16 feet to the ground below after the forks slid out from underneath the man basket. The employee sustained a broken pelvis, broken arm, and internal bleeding.
