29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a)  The employer did not ensure that employees were prtected from falling to a lower level while working on a scaffold that was more than 10 feet above the lower level.  This violation occurred on or about November 14, 2012 when two employees were riding a scissor lift up to the roof of a four (4) story building without guardrails or other forms of fall protection.
