On or about November 14th, 2012 at worksite 4240 Greenwood Place in Kansas City, Missouri- Employee was performing mason work on a scaffold platform that was not fully planked or decked between the front uprights and the guardrail supports.       Abatement certification for this violation is not required.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.        On or about November 14th, 2012 at worksite 4240 Greenwood Place in Kansas City, Missouri- Employee was performing mason work on a scaffold and using the crossbraces as a means of access to the other elevated platforms of the scaffold.       Abatement certification for this violation is not required.

On or about November 14th, 2012 at worksite 4240 Greenwood Place in Kansas City, Missouri- Employee was performing mason work on a scaffold platform that was less than 10 feet from a secondary electrical powerline. On or about November 14th, 2012 at worksite 4240 Greenwood Place in Kansas City, Missouri- Employees was performing mason work on a scaffold and had never recieved training in regard to electrical hazards.       Abatement certification for this violation is not required.
