29 CFR 1926.601(b)(4): The employer used motor vehicle equipment that had an obstructed view to the rear which did not have a reverse signal alarm audible above the surrounding noise level or an observer signaling it was safe to back up the vehicle:    a) Jobsite, Freedom Township Wastewater Treatment Plant (Upgrades), 60 Standish Lane, Duncansville, PA: On or before 11/30/12, a Caterpillar 235C excavator having an obstructed view to the rear, which did not have an operational reverse signal alarm audible above the surrounding noise level or an observer signaling it was safe to back up the vehicle, was operating at the jobsite.

29 CFR 1926.152(e)(4): Units dispensing flammable or combustible liquids were not protected against collision damage:    a) Jobsite, Freedom Township Wastewater Treatment Plant (Upgrades), 60 Standish Lane, Duncansville, PA: On or before 11/30/12, a Highland diesel fuel tank, #74917, 500 gallon, 48 inch diameter x 68 inches long, located by the main treatment building not protected against collision.
