29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Corrugated Department, 580 Fort Pond Road, Lancaster, MA    On 11/30/12 and at times prior, employees entered the print sections of a Ward Corrugated Box Making Machine during set up with the section closing controls active and with no Energy Control Procedures in place, exposing them to crushing hazards should the sections be closed inadvertently.

29 CFR 1910.219(d)(1):     Guarding. Pulleys, that were seven (7) feet or less from the floor or working platform, were not guarded in accordance with the standards specified in paragraphs (m) and (o) of this section.     580 Fort Pond Road, Lancaster, MA    On 12/12/12 and at times prior, employees were exposed to nip points of power transmission pulleys on four machines.       Instance 1.   Employees had access to the pulley nip point at the outflow end of the Skiver Machine.       Instance 2.   Employees had access to the multiple nip points on the multiple pulleys that powered the Multi-Head Drill                          Press        Instance 3.   Employees had access to the drive pulley nip points under the in-feed table of the Universal Rotary Slotter                        (Harper Slotter)       Instance 4.   Employees had access to the roller drive pulley nip points on three Sta-Warm Glue Pots.

29 CFR 1910.147(c)(7)(iv):     The employer did not certify that employee training had been accomplished and kept up to date.  The certification did not contain each employee's name and dates of training.    580 Fort Pond Road, Lancaster, MA    On 11/30/12 and at times prior, newly hired permanent and temporary employees had not been trained or certified in the companys energy control procedures, but were assigned to production areas where energy control procedures were utilized, exposing them to  mechanical hazards.      Horn Packaging Corporation was previously cited for a violation of this occupational safety and health standard (29 CFR 1910.147(c)(7)(iv)) which was contained in inspection number 315516005, citation number 1, item number 3b, issued on May 1, 2012, at the same workplace located at 580 Fort Pond Road, Lancaster, Massachusetts.  The final order date for the citation was June 26,2012.

Instance 1.  Employees had access to multiple nip points and rotating parts on the Ward Box Making Machine.   Instance 2.  Employees had access to side and outfeed nip points/rotating parts on the Serenco Rotary Die Machine.                 Instance 3.  Employees had access to in and outfeed rotating parts on the Universal (Harper) Slotter machine.  Instance 4.  Employees had access to in and outfeed nip points on the 50 inch Printer Slotter machine.   Instance 5.  Employees had access to outfeed nip points and rotating parts on the Mehlie Die Cutter machine.  Instance 6.  Employees had access to outfeed nip points on the Rapidex Stitcher machine.  Instance 7.  Employees had access to side and outfeed nip points on the Interlake Stitcher machine.  Instance 8.  Employees had access to side and in-/outfeed nip points on the  General Corrugated Taper/Gluer Machine.   Instance 9.  Employees had access to the infeed nip point on the Signode Strapper machine.   Instance 10.  Employees had access to rotating parts on the Root Drill Press.  Instance 11.  Employees had access to the outfeed nip point on the Convoluter machine.  Instance 12.  Employees had access to in and outfeed nip points and rotating parts on the S & S Slitter machine.  Instance 13.  Employees had access to in and outfeed nip points and rotating parts on the Universal Slitter machine.  Instance 14.  Employees had access to rotating parts on the Gosiger Drill Press.
