29 CFR 1926.451(d)(12)(i):      Wire ropes clips, when used on a suspension scaffold were not installed a mininum of 6 rope diameters apart:     a)  Wire rope clamps were not installed at least 6 rope diameters apart on the scaffold's suspension cable attached to the outrigger beam.    **In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).**

29 CFR 1926.451(g)(3)(iv):      Vertical lifelines, independent support lines, and suspension ropes shall not be attached to each other, nor shall they be attached to or use the same point of anchorage, nor shall they be attached to the same point on the scaffold or personal fall arrest system:     a)  Employer allowed its employee to erect and use a vertical lifeline that was anchored to a single anchorage point.      b)  Employer allowed its employee to erect and use a single vertical lifeline to support two personal fall arrest systems.     **In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).**

29 CFR 1926.501(b)(1):      Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     a)  An employee was not provided with fall protection and was exposed to the hazard of falling while working and traveling at the roof edge of the building.  The employee was exposed to a fall of approximately 70 feet to the concrete ground.      **In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.**
