29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) North End of Colliseum - On or about November 30, 2012 an employee was standing on top of concrete formwork at a height of 12 feet above the floor and he was not protected by guardrail systems, safety net systems, or a personal fall arrest system. CONCRETE CONSTRUCTION INNOVATIONS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314951039, CITATION NUMBER 01, ITEM NUMBER 001 ISSUED ON 10/29/2010, WITH A FINAL ORDER DATE OF 11/18/2010 WITH RESPECT TO THE WORKPLACE LOCATED AT 101 HWY 11, POPLARVILLE, MS.
