29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) At 2000 West Beach Boulevard Gulf Shores, Alabama: On December 12, 2012; the employer failed to ensure emlpoyees working from an aerial lift were protected from fall hazards in that he allowed them to work in the elevated aerial lift without wearing a harness exposing them to falls of 14 feet to the ground below.
