29 CFR 1926.451(f)(13):  Debris was allowed to accumulate on scaffold platforms:    65 Creasy Road, Canterbury, CT 06331, on or about 12/11/2012: The employer did not ensure that discarded roofing materials were removed off scaffold platform. An employee who was on the scaffold platform slipped on the discarded roofing materials and fell about 12 feet to the lower surface.

The roof was 18: 12 pitch.
