29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) American Hood Systems, Inc., Elyria, Ohio: On or about December 13, 2012, the employer did not ensure that each employee who operated the MEGA Manufacturing Inc. manufactured Piranha 175 Ton hydraulic press brake was protected from crushing hazards. The point of operation was not guarded to prevent employees from accessing the hazard area, which could result in amputation injuries.
