29 CFR 1926.1053(b)(6):  Unsecured ladder(s) were used on surfaces that were not stable and level:     a)  At the site, on December 18, 2012, where the employee stood on a Werner A-frame step ladder that was positioned on top of a stack of sheetrock material while installing an air conditioning refrigerate line in a newly constructed building.  This condition exposed the employee to a fall hazard.
