29 CFR 1926.20(b)(2): The employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were conducted by a competent person:  Jobsite: Lower roof skirt segment.  An employee fell approximately 13 feet onto the ground and was seriously injured while attempting to install rolled-rubber ice and water shield on a lower skirt-roof where ice had formed on the roof sheathing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Jobsite: Lower roof skirt segment.  A worker slipped and fell approximately 13 feet to the ground below while performing roofing activities where he slipped on ice that had formed on the plywood sheathing.
