29 CFR 1917.42(e)(1):     Natural and/or synthetic rope having abnormal wear was not removed from service.      Horizon Terminal, Automobile Carrier Processing:  On or about 26 December 2012 and times prior thereto, employees lifting and lowering automobile carrier top stanchions with block and tackles having ropes with abnormal wear.  This condition exposed personnel to struck by hazards.

29 CFR 1917.112(b)(1):     Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water.    Horizon Terminal, Automobile Carrier Processing:  On or about 26 December 2012 and times prior thereto, employees accessing vehicles on carriers were stepping over sixteen to eighteen inch open gaps between the carrier and the walkway.  This condition exposed employees to an 11 foot fall hazard.
