29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.   a) The employer did not conduct performance evaluations (driving tests) for all employees authorized to operate powered industrial trucks at the Woodridge facility.     There is no abatement documentation or certification required for this item.   Abatement Note:  Powered industrial truck operator performance evaluations are required every three years.

Production Area. The employer did not complete certification records of the monthly inspections of the hoist chains, including end connections, for excessive wear, twist, distorted links or stretch.    There is no abatement documentation or certification required for this item.
