29 CFR 1910.132(d)(1):     The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):     840 Sterling Rd, South Lancaster, MA      As of 1/14/13, the employer had not accomplished a workplace hazard assessment to determine whether hazards were present and personnel protective equipment was required.  At least one tool was used by employees that required eye protection per the manufacturer.

29 CFR 1910.176(b):     Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    840 Sterling Rd, South Lancaster, MA    On 1/14/13 and at times prior, tiers of lumber were not stacked in a stable manner, resulting in one stack falling into an aisle way during the inspection, one stack that had fallen just prior to inspection, and multiple lumber stacks over eight feet high that were leaning precariously.

29 CFR 1910.178(m)(5)(iii):     Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have load engaging means fully lowered:      840 Sterling Rd, South Lancaster, MA      On 1/14/13, a powered industrial truck (PIT) operator on more than one occasion stood on the front fender of the PIT while the load was elevated.  Also two PITs with the operator dismounted and within 25 feet of the truck had forks and loads elevated while employees moved lumber from one to the other.
