29 CFR 1904.41(b)(2): The OSHA Annual Survey Form was not completed and sent to OSHA within 30 calendar days, or within the time stated in the Survey Form, whichever is later.    At the facility: Double D Hook-N-Go failed to complete and send to OSHA, or its agent, the OSHA Occupational Injury and Illness Data Collection Form, 2011 within 30 days of the date Double D Hook-N-Go received the Form, which was mailed to Double D Hook-N-Go on or about June 2012.    Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
