This violation occurred on or about January 12, 2013, on the Delayed Coker Unit (DCU) where employees and contract employees were exposed to thermal hazards when performing maintenance and service tasks without affixing a lockout or tagout devices on the hand wheels valves when shutting down the DCU.       Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that lockout or tagout devices are affixed to each energy isolating device.

This violation occurred on or about January 12, 2013, on the Delayed Coker Unit where contract employees were exposed to thermal hazards when performing maintenance tasks without notification that locks were removed by opening valves such as, but not limited to, XV-323 to line up steam for chemical cleaning.      Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees are safely positioned before lockout or tagout devices are removed.
