Section 5(a)(1) of the Occupational Safety and Health Act of 1970:   The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in-between hazards:   On or about 12/25/2012, at the above addressed jobsite, an employee working in the capacity of a spotter during a rigging operation was exposed to caught-in-between hazards, whereas the cribbing underneath the spreader beam was not properly assembled causing the beam to shift during hoisting and fall onto the employee's leg.
