29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that that:    On or about 01/29/2013, at the jobsite at 1222 Spruce Street, St Louis, MO 63103, employees were engaged in capping off the end of a water pipe in a trench excavation that was 5' 7" deep.  Specifically, the employees were not provided with either sloping, benching, or a shield system to protect them against the hazard of a cave-in.
