(a) On the roof employee(s) using a pneumatic nail gun to nail down decking were exposed to eye injuries in that the nail gun operator was not wearing personal protective equipment such as but not limited to safety glasses.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) At site employees working within 2' 8" of a 120 volt power line are exposed to electrocution hazard. Employees were removing tiles from a 4/12 pitch roof.
