29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 2/07/2013, in the machine shop, the employer allowed employees to use the Sharp lathe ( model number 26160B ), Vanguard lathe (model number CA6250B) , 17" Turnnando lathe (model number unknown), Manford milling machine (model unknown) and the Webb milling machine (model unknown) to fabricate metal parts without a point of operation guard.

29 CFR 1910.184(e)(3)(i): A thorough periodic inspection of alloy steel chain slings was not made on a regular basis and/or were done at intervals greater than once every 12 months: On or about 2/7/2013, the employer allowed employees to use the uninspected chain slings located in the shop area. The employees use the chain slings to move faabricated metal around the shop area.
