These include but are not limited to, the contact cement spraying operation to determine potential fire hazards with the process and location, the employee exposure to Wilsonart 600 and 800 series adhesive, and that the appropriate personal protective equipment is available and is being used. 29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) Shop areas: On February 6, 2013 and at times prior thereto employees who voluntarily wore N95 respirators (dust mask) while performing cutting activities on materials which included, but were not limited to the acrylic countertops, were not made aware of the information contained in Appendix D. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative:    a) Shop area: On February 6, 2013 and at times prior thereto an energy control program, which would include specific written energy control procedures, effective training for authorized and affected employees, and periodic inspections of the program and procedures, had not been established or implemented.  Employees routinely perform scheduled and unscheduled service and maintenance on shop machinery.  This machinery includes, but is not limited to the horizontal beam saw and Western Automation post forming cutting station saw, CS5840. Servicing and maintenance activities include, but are not limited to changing saw blades and air lines.       Note) Abatement certification IS required for this item.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) Laminate shop area, behind horizontal beam saw: On February 6, 2013 and at times prior thereto scrap wood bins and other shop items were stored in the area around the portable fire extinguisher, obstructing clear access to it.    b) Shop area, near break room: On February 6, 2013 and at times prior thereto recycle bins were located in front of the portable fire extinguisher, blocking clear access to it.     c) Solid surface/acrylic shop: On February 6, 2013 and at times prior thereto scrap wood and other materials were allowed to accumulate in the area below the portable fire extinguisher, obstructing clear access to it by creating a trip hazard.
