29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Jobsite, Six Mile Run Area VFD, 1100 Six Mile Run Road, Six Mile Run, PA: On or before 02/12/13, employees were engaged in roofing activities on a 3/12 low-slope, 54 foot wide roof. The employees were exposed to a 21 foot fall hazard. Don Miller, Inc., was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA Inspection number 662899, Citation number 1, Item number 1 , issued on 10/12/2012, with respect to a workplace located at 2185 McClellandtown Road, Masontown, PA.

Employees were working above him on the the 3/12 pitch roof, 21 feet above the ground level, installing a metal roofing system.

When working over or near water, the requirements of 1926.106 apply. a) Jobsite, Six Mile Run Area VFD, 1100 Six Mile Run Road, Six Mile Run, PA: On or before 02/12/13, an employee was installing metal siding working from a BM personnel platform, lifted by a RO-RO truck crane, TC2863.The employee was elevated approximately 11 feet from the ground level and not using a personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Jobsite, Six Mile Run Area VFD, 1100 Six Mile Run Road, Six Mile Run, PA: On or before 02/12/13, employees were using an unsecured Werner aluminum 40 foot extension ladder, Type 1A, D1540-2 to access the roof 21 feet from the ground level The ladder rails extended approximately 1 foot 6 inches above the upper landing surface being accessed.
