29 CFR 1915.89(e)(2):     The employer did not ensure that the machinery, equipment, or systems were shut down according to the written procedures the employer established.    OSI Dock, F/V Frontier Explorer; on or about 9 February 2013 and times prior thereto, the employer did not ensure that the machinery, equipment, or systems were shut down according to the written procedures the employer established. No specific written lockout/tags-plus system procedures were available for the Technotherm ammonia pumps, Mycom ammonia compressors and the haul station block. This condition exposed employees to a caught-in hazard.
