29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)  On or about February 13, 2013, employees were exposed to injuries from falls, while installing snow stops, on a commercial building, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:    (a)  On or about February 13, 2013, the employer did not protect workers from being exposed to fall hazards while using portable ladders that did not extend at least 3 feet above the upper landing surface being accessed.
